There are standardized Medicare Supplement plans in most states (excluding MA, MN, WI). Those are plans A – G, K, L, M, N.

Medicare Supplements must cover these BASIC BENEFITS:
Medical Costs
Covers the Part B coinsurance (generally 20% of Medicare-approved payment amount) or co-payment amount which may vary according to the service.
For hospital outpatient services, the co-payment amount will be paid under a prospective payment system. If this system is not used, then 20% of eligible expenses will be paid.
Inpatient Hospital Care
Covers the cost of Part A coinsurance and the cost of 365 extra days of hospital care during your lifetime after Medicare coverage ends.
Blood
Covers the first 3 (three) pints of blood each year.

+ Foreign Travel Emergency (up to $50,000)
Plan K includes:
Basic Benefits & coverage for:
+ 50% of Part A: Inpatient Hospital Deductible ($1,364)
+ 50% of Part A: Skilled Nursing Facility Coinsurance
+ Out-of-pocket limit ($5,560) after you meet this limit and Part B deductible, the plan pays 100% of covered services
Plan M includes:
Basic Benefits & coverage for:
+ 50% of Part A: Inpatient Hospital Deductible ($1,364)
+ Part A: Skilled Nursing Facility Coinsurance
Plan F* includes:
Basic Benefits & coverage for:
+ Part A: Inpatient Hospital Deductible ($1,364)
+ Part A: Skilled Nursing Facility Coinsurance
+ Part B: Deductible ($185)
+ Foreign Travel Emergency (up to $50,000)
+ 100% coverage Part B: Excess Charges
*Has a High Deductible Option:
Pay $2,300 out-of pocket per year before the plan pays anything. Insurance policies with a high deductible option generally cost less than those with lower deductibles. Your out-of-pocket costs for services may be higher if you need to see your doctor or go to the hospital

+ 75% of Part A: Skilled Nursing Facility Coinsurance
+ Out-of-pocket limit ($2,780) after you meet this limit and Part B deductible, the plan pays 100% of covered services
Bundle with a Prescription Drug Plan
